Overview

Revision Rhinoplasty/Redo Rhinoplasty is the cosmetic or plastic surgery that, in media culture, is simply called a nose job. Plastic surgeons operate on your nose to change its shape or correct a deformity. However, as with any cosmetic surgery, the outcome is hard to predict. You may end up with a nose shape different from what you envisioned. An inexperienced surgeon may also botch up the surgery. This can cause a deviated nasal septum, collapsed or deficient cartilages or persistence of the original medical problem. In either case, the next step for most patients is revision rhinoplasty. Revision rhinoplasty aims to correct the mistake made in the primary one.

Procedure on treatment

Revision rhinoplasty is usually an open rhinoplasty, performed under local or general anaesthesia. The surgeon cuts the skin at the base of the nasal columella. He then raises the skin to expose the bone and cartilage underneath. If the nose needs reduction, the surgeon will shave away some of the cartilage and bones. If too much cartilage was removed in the previous surgery, collapse of the nasal structure may occur. This cartilage deficiency needs supplementation by cartilage grafts. The surgeon sources this donor cartilage from other body parts. This can be the ear, the temporalis fascia or even the ribs.

The nose may also need soft tissue filling to correct its shape. This too comes from another body part. A major cause of the failure of the primary rhinoplasty could be scarring. Excessive scarring can cause blockage of the respiratory pathway or asymmetrical nose shape. In either case, the dermatologist removes the scar tissue. He then uses skin grafts to prevent repeated scarring. Once the revision is over, the surgeon puts the top skin layer back in place and stitches it up.

Preparation to do Before the Treatment

You must not get a revision rhinoplasty until the wounds from the primary one are completely healed. Allow at least a year between the two surgeries. Otherwise, complications may arise.

If you are going for a new dermatologist, do your research well. Not all surgeons specialise in facial reconstruction surgery. On top of that, rhinoplasty is a complex procedure, and revision rhinoplasty even more so. Make sure the prospective surgeon has ample experience in this field.

Always go for a detailed consultation before the procedure. Let the surgeon know exactly what you want so that there are no more mistakes. Give all your previous records to the present doctor. You will also be getting scans of your nose and blood profiling done.

Your doctor will ask you to give up smoking and drinking a month before the surgery. You will also be stopping blood thinners, anti-depressants and painkillers. Let your doctor know of any medication you are taking.

You will need to make some preparations beforehand for the recovery period. You should get lots of pillows and clean bedding, enough ice packs and anti-bacterial soap.

After the Treatment

Make sure you find out from the doctor all about self-care for a speedy and correct recovery. Often, a bad rhinoplasty is due to faulty aftercare by the patient himself or herself. Write down everything your dermatologist asks you to do and follow it to the T.

Expect bleeding, congestion and bloody discharge from your nose after the surgery. The doctor will prescribe drip pads to control the discharge. Nose drops can solve the congestion. He will also give ointments to prevent drying and crusting inside the nose. Pain and swelling are common. Ice packs and prescribed pain medications should be enough to take care of that.

Sleep with your head held up with pillows. This will help with the swelling, bruising and bleeding. Do not lift anything heavy or do any strenuous activity as you may start bleeding.

You cannot restart smoking, drinking or the prohibited medications until a month after the surgery. You will be on antibiotics, analgesics, diuretics and laxatives.

The doctor will remove your stitches in a week. You will usually be fully healed in 1-2 months.
